Thank you for choosing Spine & Neuro for your care. Prior to your appointment, you may complete the new patient forms by downloading the registration packet below.

New Patient?

Here are some helpful tips for your first visit.

• Please bring all Xrays, CT scans or MRIs to your appointment. If your films are on FINAO (PACS/imaging viewing system) you do not have to bring hard copies. Please click here or contact our office (256-533-1600) to see if your films can be accessed in our office. Download The PDF
• Insurance information and card
• Information about previous treatments and medications you have attempted for your condition
• Co-payment or minimum payment (given at appointment time) for uninsured patients
• List of questions for your physician
• Registration patient registration packet (for new patients) which may be downloaded above.

Insurance Claims

As a service to our patients, we will file insurance claims to any insurance company for services rendered. You are responsible for any unpaid balance on your account. Please read our policy concerning fees and payments. Although we will file your claim, we cannot be responsible for any claim disputes since we are not a party to your insurance contact.

If your insurance requires a written referral from your referring physician, you must bring this with you at the time of your visit. If you forget to bring the referral your visit may have to be rescheduled.
