How do I get an appointment?

You may need a referral from your physician. Note: If you have Medicaid Patient 1st, you may need to bring a written referral to your first appointment.

What do I need to bring with me?

Please bring the following:

  • Please bring all Xrays, CT scans or MRIs to your appointment. If your films are on FINAO (PACS/imaging viewing system) you do not have to bring hard copies. Please check with our office or your imaging center to see if they can be accessed.
  • Insurance information and card
  • Information about previous treatments and medications you have attempted for your condition
  • Co-payment or minimum payment (given at appointment time) for uninsured patients
  • List of questions for your physician
  • Registration patient registration packet (for new patients) which may be downloaded above.

Do I need a referral with each visit?

If you are already established with the doctor, you may need a new referral if it has been six months since your last visit or for a new problem. This will depend on the individual case.

Can I change doctors?

For continuity of care, our physicians retain the same patients. However, special concessions may be made in individual cases.
